Logic Model Template

One of the objectives of the uniform guidance focuses on performance for accountability. Logic models provide a proven strategy to plan for and demonstrate program performance, and are well-aligned with the focus of the uniform guidance and its performance-based requirements. Logic models are usually presented in the form of a chart or table that reflects the relationships between the resources, activities, outputs and ultimately, the outcomes of a program. Logic models depend upon causal relationships to demonstrate outcomes and performance.
